Abstract
Fourty-four patients with inverting papilloma were analyzed to investigate types of operation, recurrence and the relation with malignancy. Male to female ratio was 2.4 : 1. Inverting papillomas involved the maxillary sinus(90.1%) and ethmoid
sinus(63.6%), orbit(15.9%) and skull base(9.1%). We performed intranasal excision of tumor(17 cases), simple excision via lateral rhinotomy or midfacial degloving approach(10 cases), medial maxillectomy via lateral rhinotomy or midfacial
degloving
approach(11 cases), total maxillectomy with orbital exenteration(1 cases) and craniofacial resection(5 cases). Recurrence was noted in eight case(29.6%) out of twenty-seven patients treated with conservative surgery and in one case(9.1%) out of
eleven
patients treated with radical surgery. Among fourty-four cases, seven patients(15.9%) had carcinoma. Complete excision by approaches which can provide wider surgical view including the medial maxillectomy will be required to reduce the recurrence
rate.
We should bear in mind the possibility of malignancy in all cases of inverting papilloma of the nose and paranasal sinuses.
